Note: Include all required documents, including Tribal ID when submitting the Enrollment form. Visit the Eligibility Page for clarification.
Update: As of March 23 2020, The Rhode Island Indian Council and it's satellite offices implemented new procedures to comply with social-distancing orders. Individuals can now enroll online using the instructions below. You may print and send these documents to the office via postage mail/scan to email/or fax.

1) You must download the forms, complete all and send back to us. ( you can also email as an attachment.)
2) If you experience trouble opening the folder, please email your case worker and we can directly send you the necessary forms. Click to be directed to your state's office.
